As a nation, accident and crises management are not our strong suites. We witness major incidents, they are on the front burner for a while, the appropriate noises are made, then we move on.
No lessons learned, nothing restructured as a result, nothing put in place to forestall repeat incidents. Where any come into existence, they are often knee-jerk reactions that are not comprehensive or sustainable.
The fact is that the very wiring of humans lends itself to incidents (occurrence). Our actions and inactions create gaps or act as triggers that set
off incidents

If that is the case, there are two options:

✓Incident investigation
The latter is to understand the former and then loop back to it. The essence being to foster prevention as much as possible.
In carrying out incident investigations, there are many methodologies at the heart of which are these fundamentals:
°What happened?
°How did it happen?
°Why did it happen?
°Could it have been prevented?
°How could it have been prevented?
°How can a recurrence be prevented?
